Codification of the type of medication list (e.g. Active medication list, Reconciled medication list, Discharged medication list, Discontinued (Inactive) medication list, Patient generated medication list (Different nuance, needs to be reviewed by a clinician before it can be entered as an active medication list), Medications that are private and need patient consent to share, Medication administration list, Medication administration list for public health and CDC reporting (e.g., for COVID, Antibiotics for Antibiotic Stewardship), Discharged medication list (Feeds the discharge summary that contains the Discharged medication list and when transferring within a hospital from one level of care to another (intersystem), a transfer medication list is used for MR).

  • Recommendation: Advance the Medication List Type data element from Level 0 to Level 2 and final USCDI v6
  • Rationale: The PACIO Community* recommends advancing the Medication List Type data element from Level 0 to Level 2 and Final USCDI v6. This request for inclusion of the Medication List Type recommends the use of LOINC codes for active medication list, discharge medication list, medication administration list, transfer medication list, etc. to allow current Medication data class elements to be reused across all needs expressed in other Level 0 data elements, such as the Medication Administration data elements and Discharge Medications data element. This data element was tested during the PACIO Connectathons for the Standardized Medication Profile (SMP) use case. This recommendation is endorsed by PACIO Standard Medication Profile, Transitions of Care, and the Personal Functioning and Engagement subgroups.
