Social Determinants of Health (SDOH) conditions and diagnoses are SDOH-related problems, concerns, conditions, or diagnoses, often determined by screening assessments of an SDOH-related risk. The HL7 Gravity Project defined value sets to represent SDOH Problems and Health Concerns.
Implementation information for SDOH conditions and diagnoses exchange can be found in Structure and Exchange of Social Determinants of Health Information. A white paper developed by National Council for Prescription Drug Programs (NCPDP), Collecting and Exchanging Social Determinants of Health Data in the Pharmacy, provides guidance on the use of ICD-10-CM and SNOMED CT US Edition codes within the NCPDP SCRIPT and Telecommunication Standards, as well as the Pharmacist eCare Plan. |

PHIT supports ONC’s recommendation for using SNOMED CT U.S. Edition and ICD-10-CM to represent SDOH conditions and diagnoses. PHIT also recommends reviewing NCPDP’s white paper, “Collecting and Exchanging Social Determinants of Health Data in Pharmacy,” which was produced to assist in the usage of these codes within NCPDP SCRIPT and Telecommunications Standards and the Pharmacist eCare Plan.
