The Advanced Health Models and Meaningful Use Workgroup will provide input and make recommendations on policy issues and opportunities to ensure health IT supports new outcomes-focused models of health care delivery and value-based payment models through the availability of data and population-management tools. This group will leverage past work that supports health IT in support of advanced health models (e.g., Meaningful Use, Quality Measures, Accountable Care Workgroup recommendations). Examples of issues to be considered include, but are not limited to, health management tools (such as clinical decision support), care coordination across all types of providers (e.g. accountable care communities), and outcome management tools (such as clinical quality measures that matter and patient-reported outcomes).
