The Southeastern Michigan Beacon Community is using health IT to improve diabetes care in Detroit.

The Southeastern Michigan Beacon Community's Herbert Smitherman Jr., MD, Kimberlydawn Wisdom, MD, and Terrisca Des Jardins, Beacon Program Director, discuss how their Beacon Community will help increase use of health information technology through mobile devices to improve diabetes care in Detroit. Through the technology, patients will have the ability to take self-assessments and the results will direct them to resources in the community which can help them. The primary focus will be on diabetes but once the system is established it will move to other areas of diagnosis.
