How can we improve the ability of patient to convey their preferences regarding access to and use of their electronic medical record information? San Diego Health Connect – a recipient of an ONC Leading Edge Acceleration Projects in Health IT (LEAP) program award – developed a scalable consent framework for electronic health records to help answer this question.
Using Standards to Focus on Transitions of Care

Transitions of care (e.g. from a nursing home to an emergency department) are often difficult, but are even more dangerous for the patient and healthcare providers as a result of the COVID-19 pandemic. ONC Annual Meeting 2021: Perspectives on the Impact of SDOH on Health Equity and Care

Health equity and social determinants of health (SDOH) data were a recurring theme at this year’s ONC Annual Meeting. Leveraging these important data, which reflect where people are born, live, learn, work and play can have a significant impact on patient care and public health.
