The Beacon Community Cooperative Agreement Program was established to build and strenghten health IT infrastructure to support clinical transformation efforts. This brief presents findings from a series of site visits and interviews with the Beacon Communities examining their approaches to and experiences with building and strengthening their health IT infrastructures, specifically with respect to successes and challenges related to the sharing of patient-specific information to improve clinical care. .
