Social Connection is an umbrella term that encompasses the structural, functional, and quality aspects of how individuals connect to each other. Social Connection currently has three defined social risks as subsets. Social Isolation: Is objectively being alone, having few relationships, or having infrequent social contact. Loneliness Definition: Is subjectively feeling alone. The discrepancy between one’s desired level of connection and one’s actual level. Lack of Social Support: The actual or perceived lack of availability of resources (e.g., informational, tangible, emotional) from others. There are four types of social supportive behaviors: emotional, instrumental, informational, and appraisal.

The AMA requests that the Current Procedural Terminology (CPT) code set be added to the standards listed in Section I: Representing Social Connection and Isolation. The CPT Evaluation and Maintenance services include assessing a patient’s social history.
CPT is a comprehensive and regularly curated uniform language that accurately describes medical, surgical, and diagnostic services and provides for reliable communication among users. It has an extremely robust and mature development process with open and transparent meetings and clinical input from national medical specialties and relevant stakeholders. It is the most widely adopted outpatient procedure code set. Use of the CPT code set is federally required under HIPAA.
