Health IT in Long-term and Post Acute Care Issue Brief


This issue brief provides an overview of the opportunities for long-term and post-acute care (LTPAC) providers to use health information technology (IT) to improve care delivery and health outcomes while reducing total cost of care.


The material in these guides and tools was developed from the experiences of Regional Extension Center staff in the performance of technical support and EHR implementation assistance to primary care providers.
