Date of Onset
A date field associated with each diagnosis on a patients problem list and medical history to capture the date on which the signs/symptoms/pathology of the diagnosis began. One could imagine, in the future, supplementing this with additional data elements for capturing identifying information for the individual making this determination, their role (patient, caregiver, provider, etc.), institution, some categorization of the basis for the determination (symptoms, signs, laboratory abnormality, etc.)


Suggest changing the definition

MedMorph supports the addition of the Date of Onset element, but suggests a slight change to the definition. The word "capture" can be easily confused with the recorded date. Also, using "diagnosis" in the definition is more specific than intended. At this point in care, it is simply the onset of the condition or problem. Suggested Definition: The estimated date, actual date or date-time the condition began.
