Smoking Status belongs within Social History

The Smoking Status data element should be in a data class called Social History.

In general, I think the USCDI should focus more on providing clear definitions of the common data elements, and focus less on attempting to classify them.  The data elements may need to be used in many different places within the various other data classes being defined.  For example, if you define a data class of "risk factors", then smoking status may fit there too.  The more important work for establishing common data elements, is establishing a consensus on the definition for the data element.
