Within the health information technology (health IT) world, few things have been as elusive as a governance framework for nationwide health information exchange. When ONC was formed in 2004, the concept of a nationwide health information network\u2014where your information could be located across the country in a click\u2014was a big picture vision that drove the federal government\u2019s early health IT infrastructure, standards, policy actions, and investments.<\/p>\n\n\n\n<p>What\u2019s followed as a result of two laws (<a href=\"https:\/\/healthit.gov\/wp-content\/uploads\/2025\/06\/hitech_act_excerpt_from_arra_with_index.pdf\" target=\"_blank\" rel=\"noreferrer noopener\">HITECH Act<\/a> and <a href=\"https:\/\/www.congress.gov\/bill\/114th-congress\/house-bill\/34\">21<sup>st<\/sup> Century Cures Act<\/a>) and remarkable continuity across four administrations has been steady growth, maturity, and investment from both the public and private sectors. Most health care providers use electronic health records.<a href=\"#one\"><sup>1,2,3 <\/sup><\/a>Many are connected to and engage in at least one form of electronic health information exchange.<a href=\"#four\"><sup>4,5,6 <\/sup><\/a>And health information networks are now dotted across the US map supporting communities with a wide range of services.<a href=\"#seven\"><sup>7<\/sup><\/a><\/p>\n\n\n\n<p><a href=\"https:\/\/www.hhs.gov\/about\/news\/2022\/01\/18\/onc-completes-critical-21st-century-cures-act-requirement-publishes-trusted-exchange-framework-common-agreement-health-information-networks.html\">Today\u2019s milestone<\/a> marks the beginning of a new era of electronic health information exchange in the US. That world we wanted to see back in 2004\u2014it\u2019s here\u2014and now it\u2019s time to put what we\u2019ve built to use.<\/p>\n\n\n\n<p>Our goals for the Trusted Exchange Framework and Common Agreement&nbsp;(TEFCA<sup>SM<\/sup>) are:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Goal 1: Establish a universal policy and technical floor for nationwide interoperability.<\/li>\n\n\n\n<li>Goal 2: Simplify connectivity for organizations to securely exchange information to improve patient care, enhance the welfare of populations, and generate health care value.<\/li>\n\n\n\n<li>Goal 3: Enable individuals to gather their health care information.<\/li>\n<\/ul>\n\n\n\n<p>The <a href=\"https:\/\/healthit.gov\/wp-content\/uploads\/2022\/01\/Trusted_Exchange_Framework_0122.pdf\" target=\"_blank\" rel=\"noreferrer noopener\">Trusted Exchange Framework<\/a> is a set of non-binding principles to facilitate data-sharing among health information networks. The <a href=\"https:\/\/healthit.gov\/wp-content\/uploads\/2022\/01\/Common_Agreement_for_Nationwide_Health_Information_Interoperability_Version_1.pdf\" target=\"_blank\" rel=\"noreferrer noopener\">Common Agreement<\/a> will operationalize simplified electronic health information exchange for many across the US and will provide easier ways for individuals and organizations to securely connect. Most notably, the Common Agreement sets a new baseline for the exchange purposes that need to be supported\u2014a common source of friction across networks today. The Common Agreement includes support for treatment, payment, health care operations, individual access services, public health, and government benefits determination.<\/p>\n\n\n\n<p>The Common Agreement is a new legal contract that ONC\u2019s Recognized Coordinating Entity<sup>\u00ae<\/sup> (RCE<sup>TM<\/sup>), The Sequoia Project, will sign with each Qualified Health Information Network<sup>TM<\/sup> (QHIN<sup>TM<\/sup>). QHINs will then execute certain corresponding policies within their own networks. Published in tandem is the <a href=\"https:\/\/rce.sequoiaproject.org\/wp-content\/uploads\/2022\/01\/QTF_0122.pdf\">QHIN Technical Framework<\/a>, which sets the functional and technical requirements that QHINs need to support to make this new connectivity come online. While road-tested production standards are being used at the start, we are also actively working with the RCE to develop a TEFCA Health Level Seven (HL7<sup>\u00ae<\/sup>) Fast Healthcare Interoperability Resource (FHIR<sup>\u00ae<\/sup>) Roadmap (<a href=\"https:\/\/rce.sequoiaproject.org\/wp-content\/uploads\/2022\/01\/FHIR-Roadmap-v1.0_updated.pdf\">TEFCA FHIR Roadmap<\/a>) to outline how FHIR will also become an established part of TEFCA-based exchange over time.<\/p>\n\n\n\n<p>Thank you to everyone that\u2019s been a part of this work over the years\u2014through the drafts, listening sessions, and webinars you have helped us shape where we have arrived at today.
