Over the last several weeks we have been following Larry (Part 1, Part 2, Part 3), a patient with diabetes and a history of non-compliance. Because Larry’s primary care providers either do not have EHRs or other care coordination tools or are unable to exchange information between one another, they do not know that he had been hospitalized and instructed to make important changes in his medications.Read Full Post.

Last week we introduced Larry, the patient with diabetes and a history of non-compliance who is discharged from his local emergency department, but his primary care doctor is never notified about his visit.
