State HIE Implementation Status

The State HIE Program launched in February 2010 and provided many grantees with funding and time to develop strategic and operational plans (SoPs) to enable statewide exchange prior to launching their implementation efforts as part of the program. By early 2012, all grantees had ONC-approved SoPs and had launched implementation activities; thus reporting of implementation progress began in Q2 2012.

The first figure on this page displays the combined directed exchange1 and query-based exchange 2 implementation status for all State HIE grantees in calendar quarter 4 of 2012. It also presents important information about each grantee, including their strategic plan model, and where they have been highlighted as HIE Bright Spots.

The second and third maps on this page present a break-out look into directed and query-based exchange, respectively, for each grantee in calendar quarter 1 of 2013. These maps display the availability of directed and query-based exchange mechanisms in each state (broadly available3, available in regions4, pilot5, or not currently available6) and vendors associated with the mechanisms reported. Directed exchange maps also display the implementation approach taken by the grantee (contractual7, marketplace8, or hybrid 9). Options accounted for in these maps include regional- and state-level entities that facilitate exchange across unaffiliated organizations; they do not account for exchange enabled by enterprise (private) HIE entities serving integrated delivery networks (IDNs) or hospital systems.

Note: The vendors attributed to the states below are based on self-reports from states. State HIE grantees contract with vendors at their own discretion. The listing of vendors on the State HIE Program Measures Dashboard and associated materials does not indicate endorsement of any specific vendor. All information contained on this page is self-reported by State HIE grantees. As the State HIE Cooperative Agreement Program period of performance has ended, the Program Measures Dashboard will not be updated after Q4 2013. This information will remain on HealthIT.gov to provide historical data on the Program.

The figure below shows each state’s query-based exchange implementation status, HIOs/HIE entities participating, and vendor(s) during the quarterly reporting period. Data include operational options that exist and that providers can subscribe to. Options accounted for may include those not directly funded or supported by State HIE grantees. Query-based exchange implementation maps are the only dashboard components that may include non-grantee funded or supported/enabled options. To access this information, click each state and a pop-up will appear.

Helpful Definitions

1Directed exchange: Point-to-point secure communication supported by the Direct Project specifications or other industry approaches to secure messaging.

2Query-based exchange: “Pull” transactions, or query through an HIE entity, allow a user to submit a request for patient information and ask the HIE entity to discover and provide any records it may have about the patient in accordance with policies governing patient consent and data use agreements between exchange users.

3 Broadly available: Regional- and state-level entities that facilitate exchange across unaffiliated organizations exist and can be subscribed to for directed exchange and/or query-based exchange. These broadly available options and services do not account for exchange enabled by enterprise (private) HIE entities serving integrated delivery networks (IDNs) or hospital systems.

4 Available in regions: Directed exchange and/or query-based exchange is available within regions of the state but not currently statewide. These options and services available in regions do not account for exchange enabled by enterprise (private) HIE entities serving IDNs or hospital systems.

5Pilot: Directed exchange is available to a limited number of providers participating in a pilot or test.
